MDS LTC Coordinator

Job Summary:

We are seeking an experienced MDS LTC Coordinator to join our team at Runnells Rehab Center in Berkeley Heights, New Jersey. As an MDS LTC Coordinator, you will be responsible for ensuring timely, accurate, and complete assessments of residents' health and functional status during the entire assessment period. You will also ensure accurate and timely completion of all Medicare/Medicaid case-mix documents and other regulatory requirements.

Responsibilities:

• Ensure timely, accurate, and complete assessments of residents' health and functional status during the entire assessment period

• Ensure accurate and timely completion of all Medicare/Medicaid case-mix documents

• Perform concurrent MDS review to assure appropriate RUGS category is achieved through the capture of appropriate clinical information

• Identify opportunities to appropriately enhance reimbursement

• Maintain an accurate schedule of all MDS assessments

• Conduct regular audits of MDS process, including validation of coding documentation, evaluating outcomes, and utilizing Data Integrity Audit reports

Job Requirements:

• Bachelor's degree in Nursing, Health Information Management, or related field

• Strong knowledge of Medicare and Medicaid regulations, including RAI, MDS, and OASIS

• Excellent communication and interpersonal skills

• Ability to work independently and as part of an interdisciplinary team

• Strong analytical and problem-solving skills
