Full Time Physical Therapist Case Manager Anticipated Territory: Rio Vista Area In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, enabling them to live safely and productively in their own environment. Your goals as a Physical Therapist are to restore function to prevent or minimize disability following disease or injury and help clients reach their maximum performance within their capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs and may supervise Physical Therapy Assistants (PTAs) in accordance with agency and regulatory standards. What you’ll do as a Home Health Physical Therapist Case Manager Perform an evaluation in the home and develop a care plan, coordinating with PTAs to follow the plan. Help patients with mobility, movement, and regaining independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure a quality level of care. Evaluate and report patients’ progress to MD. To be a success as a Physical Therapist Current unrestricted CA Physical Therapist License. Current BLS/CPR Certification. Valid CA Driver's License. Knowledge of local community resources. Knowledge of individual development and human behavior relative to illness/injury and cultural influences on health care. Knowledge of state and federal homecare regulatory guidelines preferred. Experience in home health care highly preferred. Experience with high acuity patients a plus.Experience working with an interdisciplinary team highly desired. Wage range: Physical Therapist, Level 1: $120,000–$135,000 plus $10K annual bonus, paid quarterly. Starting pay commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (up to 4% match).
