Key Responsibilities\n

• Conduct comprehensive psychosocial assessments by interviewing patients, observing their interactions, and reviewing medical records to identify emotional, social, and environmental factors affecting their recovery process.

• Collaborate with the multidisciplinary rehabilitation team to create and implement tailored treatment plans that address the unique psychosocial needs of each patient.

• Educate patients and their families about community resources that may aid in their recovery, such as support groups, vocational rehabilitation, or housing assistance programs.

• Actively participate in discharge planning, ensuring patients and families are well-equipped with knowledge and access to resources post-rehabilitation.

• Utilize electronic health record systems to document interventions and track patient progress toward rehabilitation goals.

Your work will focus on a diverse range of patients—including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders—each with unique challenges. Qualifications\n
