Key Responsibilities\n

• Conduct thorough clinical evaluations to assess patients’ mental health needs accurately.

• Lead therapy sessions and psychoeducational groups, facilitating discussions and activities that promote healing and coping strategies.

• Implement case management practices, beginning discharge planning upon patient admission to ensure seamless transitions.

• Provide crisis intervention in both individual sessions and within the treatment milieu, demonstrating effective de-escalation techniques.

• Collaborate with the Utilization Review and Business Departments to manage patient authorization and ensure financial procedures align with care plans.

• Facilitate regular communication with referral sources and outpatient providers to maintain continuity of care, addressing any concerns regarding a patient’s treatment plan.

• Participate in clinical outreach and program tours, representing our organization at events and contributing to community awareness initiatives.
\n
• Support milieu staff by providing guidance during patient interactions, helping to foster an environment of trust and respect.

• Assess patients for risk factors, including suicidality, to ensure patient safety at all times.
\n
• Document service delivery accurately and in a timely manner, including initial assessments, treatment plans, and clinical notes, utilizing Office 365 and Electronic Health Record systems.
