Key Responsibilities\n

• Independently manage a designated caseload of patients, comprehensively assessing their social, emotional, and financial circumstances to inform personalized treatment plans.

• Utilize various therapeutic modalities to mitigate psychosocial barriers, ensuring that patients are prepared for successful transitions within their care continuum.

• Collaborate with healthcare teams to advocate for patients, addressing protective services and life care decisions while facilitating advanced directives.

• Provide crisis intervention, educational resources, and referrals to patients and their families, ensuring their psychosocial needs are seamlessly integrated into treatment planning.

• Supervise and mentor Social Workers, MSW students, and interns, fostering a collaborative learning environment and promoting best practices in social work.

• Lead coordination efforts to prevent hospital readmissions by connecting patients with community resources that support their ongoing care and rehabilitation.

• Respond efficiently to patient crises and psychosocial service requests, ensuring timely execution of treatment plans and adherence to discharge goals.

• Participate actively in team rounds and patient care conferences, sharing insights and recommending best practices based on your evaluations and experiences.

• Convene with multidisciplinary teams and community agencies to enhance integrated care delivery and strengthen community resource connections for patients.

Qualifications\n

• Master’s Degree in Social Work (MSW) from an accredited institution.

Compensation and Benefits\n

This position offers a competitive annual salary ranging from USD 120,000 to 130,000, commensurate with experience.
