Bilingual Community Health Partner: Spanish/Chinese Outreach

Raising Health is looking for a dedicated Community Health Partner (CHP) to support families in Brooklyn, NY. This full-time role requires bilingual skills in either Spanish or Chinese and involves providing education and case management across various social services. With a focus on outreach and community engagement, the CHP will play a critical role in guiding families in accessing essential resources. A commitment to in-person service is essential, working from Tuesday to Saturday, 10am to 6pm.
