Home Health Aide Job Description Template
Our company is looking for a Home Health Aide to join our team.
Responsibilities:
- Supervise physical and mental patient health;
- Effectively communicate any dietary or medical needs to family and staff;
- Light housekeeping tasks, laundry and meal preparation;
- Alzheimer’s Care;
- Assists with respite and companion services;
- Attempts to promote client’s mental alertness through involvement in activities of interest;
- Gives emotional support to the patient as outlined in the Patient Care Plan;
- Back rubs;
- Assist client in walking and transportation to doctor appointments, outdoor activities, shopping;
- Establishing and maintaining effective communication and a professional relationship with client’s, family members, and co-workers;
- Reminds client to take self-administered medications;
- Maintains notebook or folder with patient individual aide assignment forms for each patient seen by the HHA;

- Supervise physical and mental patient health of our clients;
- Keeping the patient’s living area clean and orderly as appropriate.
Requirements:
- Must be 18 years of age;
- Up-to-date car insurance in your name, registration and valid Driver’s License;
- Unencumbered and valid Home Health Aide certification;
- Valid driver’s license;
- Must be a Certified Nursing Assistant or have 1 year experience in homecare;
- Authorized to work lawfully in the United States;
- Reliable, energetic, self-motivated and well-organized;
- Must have reliable transportation;
- Successful completion of a nursing assistant program;
- Requires knowledge of principles of good personal hygiene and aseptic techniques;
- High School Diploma or equivalent/Experience as a Caregiver;
- Must be reliable and professional;
- Health Statement/Physical exam within 6 months of hire;
- Current TB test and Physical exam;
- Work experience/Certification as a health aid/CNA (6 months perferred).
