Responsibilities:

The Program Supervisor Wound & Vascular Access will oversee the Skin and Would Assessment Team (SWAT) and IV Vascular Access program at Fox Hospital and Fox Nursing Home. This role will develop policies, procedures and national standards of care based on national benchmarks in SWAT and IV Vascular Access nursing. This role will oversee education, hiring, disciplinary actions and decision making of both programs. This role will monitor and report any hospital acquired infections and put into place action plans events and occurrences of incidents and maintain both programs to maintain regularity compliance with state, local and federal guidelines. This role will prepare operational and capital budget and submit variance reports to executive leadership. This role will also do auditing and develop policies and procedure improvements.

The Program Supervisor- Wound and Vascular Access delivers specialized direct patient care to patients with acute or chronic wounds or ostomies, and acts as a consultant and educator to both patients and families and healthcare professionals. Collaboration with nurses, physicians and other healthcare providers to plan patient care. The Program Supervisor- Wound and Vascular Access works autonomously as well as collaboratively in the acute care setting and the Fox Nursing Home. Follow-up with patients may include ambulatory nurse visits and assistance with provider visits in the ambulatory
clinic setting.

The Program Supervisor- Wound and Vascular Access assesses, plans, delivers, coordinates, integrates, and evaluates nursing care related to patient needs at the expert level. In addition to direct patient care, responsibilities include education of colleagues regarding effective wound and skin care, ostomy care, and pressure ulcer prevention strategies. The Program Supervisor- Wound and Vascular Access contributes to the support of patients through interdisciplinary teamwork, assures that the care provided is consistent with applicable standards, regulations, statutes and state and federal law, and provides leadership to staff, serving as a resource and mentor. This role contributes to the body of professional nursing practice through participation in research, committee work, and professional organizations, internal and external.

The Program Supervisor - Wound and Vascular Access will maintain and insert central line and peripherally inserted catheters by ultrasound. This role will act as a consultant to patients, families and healthcare professionals. Collaboration with nurses, physicians and educator to patient, families and other healthcare nurses and physicians to plan patient care. The Program Supervisor- Wound and Vascular Access works autonomously as well as collaboratively in the acute care setting and the Fox Nursing Home. Follow-up

Provides nursing care and services to wound and ostomy patients throughout the life span and in various settings at an expert level consistent with experience and educational preparation.
Collects, analyzes, and records relevant patient health data accurately, for patients with wounds, skin problems or ostomies,
Analyzes data and performs expert-level assessments of wounds or ostomies to identify the patients needs and problems.
Identifies expected goals and outcomes individualized to the patient and communicates the goals effectively with all team members.
Develops a plan of care that describes interventions to attain expected goals and outcomes and communicates the plan effectively with team members.
Implements and records interventions appropriate to patient diagnosis or problem, utilizing a working knowledge of all wound, ostomy and continence products, devices and treatment methods, as well as the use of wound photography.
Evaluates the patients progress toward attainment of identified goals.
Evaluates overall outcomes for groups of patients and provides data analysis and leadership
within groups such as the pressure ulcer prevention work group.
Adheres to approved, research-based practice standards, and demonstrates awareness of national guidelines.
Documents patient notes that demonstrate expertise and clearly evaluate the patients status and progress toward achievement of goals related to individualized care.
Demonstrates consideration of safety, effective utilization of equipment and materials, and cost in managing patient care
Provides education and mentoring to healthcare staff members or students.
Assesses knowledge of nursing and provider colleagues related to wound and ostomy
management and plans, implements and evaluates learning activities to meet identified educational needs.
Assesses knowledge of nursing and provider colleagues related to pressure ulcer prevention and plans, implements and evaluates learning activities to meet identified educational needs.
Provides individualized teaching and mentoring during the provision of direct patient care to students or colleagues.
Coaches and mentors identified resource nurses (skin champions) from each acute care unit and nursing home.
Advocates for patients to ensure that care preserves and protects the patients autonomy, dignity and rights.
Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity.
Advocates for patient needs as demonstrated by patient or coworker feedback or written exemplar or managerial observation.
Adheres to ANA Code of ethics for Professional Nurses.
Meets mandatory requirements.
Maintains current BLS.
Completes and maintains specific clinical competencies for this role, including certification in wound, ostomy nursing.
Performs similar or related duties as requested or directed.
Vascular Access Specialist
Provides nursing care service throughout the life span and various settings consistent with the experience and attainment of clinical proficiency and educational preparation in the field of intravenous therapy and blood administration.
Successfully inserts IV catheters in a safe and timely manner on all age groups that are noted to have difficult access.
Adheres to CDC and INS standards in all insertions and care of all intravenous lines.
Analyzes assessment data to identify patients problems with intravenous appropriate to the need of peripheral access or central line access to complete therapy.
Collects, analyzes and records relevant patient health data accurately on weekly worksheets and documents, if necessary, in the patients chart.
Demonstrates consideration of safety and adheres to hospital policy regarding blood
administration.
Adheres to the care of central line dressing per CDC and infection control policies.
The Vascular Access Nurse shall be trained and complete certification to insert ultrasound peripherally inserted central catheters and become proficient.
Inserts peripheral intravenous catheters for intravenous access. Blood draws when inserting IV catheters on specific clients with documentation on daily work sheets.
Insert peripheral intravenous catheters for intravenous access on clients that are noted to have difficult access in all age groups in a safe and timely manner.
Maintains aseptic technique as described in CDC and INS standards of practice.
Able to maintain communications with nursing staff regarding integrity, length of time and general condition of each clients device.
Assist in the administering of blood and blood products per hospital policy.
Start the process of becoming certified and validated to place peripherally inserted central catheter using the ultrasound method.
Maintain central line dressing changes as per CDC and Infection Control policy.

Qualifications:

Education:

4 Year / Bachelor's Degree, Graduation from an accredited Baccalaureate, Professional Nursing Program, required
Master's Degree Masters degree in Nursing, preferred

All RN's hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor's prepared nursing program within 4 years of hire. BSN must be completed within 10 years. RNs licensed on or before 12/19/17 are/remain exempt from this requirement

Experience:

With No degree:
- Minimum 5 years RN experience, required
With BSN or MSN:
- Minimum 5 years RN experience and 2 years leadership experience, required

Licensure/Certifications:

Current NYS license as a RN for all education levels, required
BLS certification for all education levels, required
WON and WOCN certification within 2 years, required
