Description

Hennepin County's Public Health & Clinical Services Health Care for the Homeless (HCH) Program is seeking a Peer Support Specialist to join our Medical Respite Team. The main goal of the peer support specialist is to provide support to clients in the medical respite program and facilitate access to care and resources. This position will also provide support to clients in their health care and recovery journey and support harm reduction principles and practices in HCH's respite program to align with Hennepin County's mission of healthy communities. The Endeavors respite program is a short-term service for patients experiencing homelessness who are too sick to be on the streets or shelter, but not sick enough to be in the hospital. Respite provides a space for patients to recover from an acute health concern or exacerbations of chronic health conditions. While in respite wraparound services are provided by an interdisciplinary team consisting of registered nurses, nurse practitioners, licensed independent clinical social workers, and pharmacy and peer support specialists. Location and hours:
This position is hybrid and will be located at HCH's Endeavors respite program, at 1414 11th Avenue, South Minneapolis, MN 55404 and remote as job duties require. Work hours are Monday through Friday, 8 a.m. - 4:30 p.m. with occasional evening coverage, as needed. No holidays or weekends. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment. In this position, you will:

Provide care coordination which may include driving clients to medical appointments, assisting them with obtaining vital documents, helping them apply for county, state, and federal benefits, and coordinating referrals to housing and other social services to a caseload of clients who are experiencing homelessness and participating in the medical respite program.
Maintain contact and act as liaison with the providers of service and/or social service agencies in the community regarding client health and issues of health in the homeless service provider community.
Attend community meetings and represent Health Care for the Homeless and Public Health in collaborative or case consultation meetings in Hennepin County.
Implement and coordinate specifically assigned care coordination tasks, such as teaching clients how to schedule medical rides and appointments, contacting their pharmacy to refill medications, and provide appointment reminders.
Co-facilitate recovery groups, health education and counseling, provide targeted outreach to program participants, and delivery medication as needed.
Consult and participate in an interdisciplinary care team to ensure that health needs and care plan of clients are met and communicate regularly and directly with the team in person and document all activities in Epic.
Participate in special quality improvement projects and activities and recommend changes in the existing health-related social service programs, policies, and procedures.

Need to have:

One of the following:
- Two years or more of experience working in a community outreach or public health setting.
- Completion of two years or more of college, in social/behavioral sciences or another field related to the area.
A valid driver's license, access to reliable transportation to travel to community sites, shelters, and drop-in centers throughout Hennepin County, and the ability to obtain a Hennepin County driver's permit.

Nice to have:

Completion of a peer support certification program.
Experience with homelessness, mental health, and/or substance use.
Knowledge of:
- Community resources and social services available to clients.
- Harm reduction and recovery practices.
Ability to:
- Show patience, compassion, and empathy, and listen to the member's needs.
- Handle several different problems and assignments, simultaneously.
- Identify members/clients' concerns and/or needs.
- Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
- Work both independently and collaboratively as an effective team member.
- Use Epic or other similar computer data systems to perform data entry where accuracy is essential.
