Job Description WOOSTER COMMUNITY HOSPITAL
Social Worker: Full-Time

Provides direct social work services to patients and families identified as having complex needs. Intervenes as necessary with patients seeking emergency care or other ambulatory care services. RESPONSIBLE TO: Manager, Care Management

Bachelor’s degree in Social Work (BSW); Master’s degree in Social Work (MSW) preferred .
Minimum of two years of experience in a medical setting.
Strong knowledge of social work principles, patient advocacy, and community resources.
Master’s degree in Social Work (MSW) from an accredited program.
Familiarity with Meditech an electronic health record (EHR) and case management software such as Careport.
Training or certification in trauma-informed care or crisis intervention.

Conduct thorough psychosocial assessments of patients and their families to identify needs and barriers to care.
Formulates and completes discharge plans for patients of all ages in collaboration with physicians and other disciplines participating in the care of the patient.
Develop and implement individualized plans of care in collaboration with health care providers and community resources.
Provide supportive counseling and crisis intervention to support patients facing emotional, social, or financial challenges.
Advocate for patient access to necessary services, including housing, financial assistance, and mental health support.
Maintain detailed documentation of patient interactions, care plans, and progress in compliance with regulatory standards.
Collaborate with interdisciplinary teams, including the RN CM, to coordinate care and ensure continuity of services.
Monday - Friday 0800-1630;
Part of Saturday Rotation
