As a Licensed Social Worker at Kenton Nursing & Rehab Center, you will play a vital role in providing holistic care to our residents, addressing their emotional, social, and psychological needs. Your expertise will enable you to develop and implement individualized care plans, facilitate communication with residents, families, and interdisciplinary teams, and ensure that each resident receives the highest level of care and support.

Assess the emotional, social, and psychological needs of residents and develop individualized care plans to address these needs
Provide counseling, therapy, and support to residents and their families as needed
Conduct social worker assessments and make recommendations for care plan changes as needed
Document resident information accurately and thoroughly, maintaining confidentiality and adherence to HIPAA guidelines
Participate in staff meetings, training, and education programs to stay current with industry best practices and regulations
Collaborate with other healthcare professionals to ensure that residents receive comprehensive care and support
