LPN Licensed Practical Nurse - Immediate Care Clinic

Licensed Practical NurseFairview has an immediate opening for a Licensed Practical Nurse to join our Dermatology team in Bloomington, MN. Clinic Hours of Operation are Monday-Thursday: 7:00AM – 6:00PM and Friday: 7:00AM – 5:00PM.The LPN performs delegated nursing and medical functions of data collection and delivery of care to an assigned group of patients, or individualized patient. The LPN works under the direction of a Registered Nurse, or physician/authorized practitioner and in collaboration with other members of the healthcare team. The LPN is responsible for adhering to all policies, procedures, and nursing practice guidelines.ResponsibilitiesCollects data for patient assessments.Involves the patient, family, and other health team members in gathering relevant data.Documents data collected in the medical record.Reports unusual data or changes in patient condition to the RN, or physician/authorized practitioner in a timely manner.Delivers care as delegated by the RN or physician/authorized practitioner to assigned patient population, or individual patient.Follows the patient's plan of care.Prioritizes work based on patient acuity, available resources, and patient/family preferences, in collaboration with the RN or physician/authorized practitioner.Performs high and low risk technical procedures, including medications, defined by the clinical setting.Documents care in the medical record.Effectively communicates compliance and concerns to delegating RN or physician/authorized practitioner.Takes action to assure continuity of care for all assigned patients.Adheres to all relevant policies, procedures, and nursing practice guidelines.Communicates effectively with other members of the health care team.Documents care in the medical record.Required QualificationsVocational School orAssociates Degree in NursingMN Licensed Practical Nurse (LPN)Preferred Qualifications1 yearPrevious ambulatory Care experience.Fairview offers a generous benefit package including but not limited to medical, dental, vision plans, life insurance, short-term and long-term disability insurance, PTO and Sick and Safe Time, tuition reimbursement, retirement, early access to earned wages, and more!
