Description
Conduct thorough evaluations of patients in inpatient and outpatient settings, crafting individualized treatment plans tailored to their unique needs
Perform physical therapy interventions that prioritize mobility, strength, balance, and overall function
Collaborate actively with nurses, physicians, case managers, and fellow rehabilitation professionals to enhance recovery outcomes
Guide patients and their families through therapeutic goals, discharge plans, and at-home exercises, fostering understanding and confidence
Maintain meticulous documentation regarding evaluations, treatment progress, and outcomes, in line with facility standards and regulatory requirements
Engage in interdisciplinary team rounds and conferences, ensuring a coordinated approach to patient care
Education
~ Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications
Active state licensure as a Physical Therapist
CPR certification preferred
Additional specialized certifications, such as in geriatrics or neurology, are beneficial but not mandatory
