RN - Registered Nurse Weekends (Home Health)

Registered Nurse - Home Health Care
Location: Portland, Maine
Assignment Duration: This is a 13-week contract position.
Schedule: Day shift from 8:00 AM to 5:00 PM with every other weekend rotation. Permanent schedules include Tuesday to Saturday or Sunday to Thursday based on agency needs.
Job Description: The Registered Nurse will provide comprehensive home health care, managing a caseload of 20-25 patients and conducting 5-6 visits per day. The role involves collaboration and effective communication to ensure high-quality patient care.
Responsibilities:

Conduct patient assessments and develop individualized care plans.
Perform skilled nursing procedures including IV therapy, tube feeds, and wound care.
Utilize OASIS documentation to track patient progress and outcomes.
Communicate effectively with patients and their families, providing education and support.
Engage in team collaboration to improve patient care strategies.
Manage patient schedules efficiently and document encounters in the EMR system.
Required Experience / Certifications / Licensure:

At least 2 years of nursing experience, preferably in home health care.
Current RN license in the state of Maine.
BLS certification required.
Experience with Epic and OASIS documentation is essential.
Skills in IV therapy, tube feeds, and wound care treatments are preferred.
