Description As an Advanced Practice Provider (APP) – Nurse Practitioner/Physician Assistant’s play a key role in leading the care team and delivering comprehensive primary healthcare to a panel of patients. You will provide medical oversight, expertise, and leadership to ensure the delivery of high-quality, cost-effective healthcare services. Key responsibilities include:

Provide patient-centered, comprehensive primary care services consistent with a value-based care model

Conduct patient evaluations, diagnoses, and treatment plans based on evidence-based guidelines

Work collaboratively with physicians and other healthcare professionals to ensure continuity of care

Participate in quality improvement programs and contribute to the evaluation of practice performance metrics

Ensure compliance with privacy regulations, quality programs, and all HealthyU Family Medicine policies, procedures, and protocols

Utilize and implement new healthcare technologies, products, services, and programs to enhance patient care

Educate patients on preventive care, chronic disease management, and overall wellness

Maintain accurate and up-to-date patient documentation in the Electronic Health Record (EHR) system

Perform other duties as assigned

Requirements Completion of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program

Active and unrestricted state license as a Nurse Practitioner or Physician Assistant (required by the first day of employment)

Current Basic Life Support (BLS) certification

Active DEA registration

Ability to provide two professional references

Minimum of one year of clinical experience in a primary care setting

Experience providing longitudinal patient care and continuity of care

Knowledge of managed care principles, including quality assurance, quality improvement, risk management, population health, and disease management programs

Demonstrated ability to collaborate effectively within a multidisciplinary healthcare team

Excellent interpersonal and communication skills with patients, providers, and staff

Self-motivated with a patient-centered, results-driven approach to care

Bilingual in English and Spanish strongly preferred
