Actify Home Care is clinician-owned and operated by a Physical Therapist with over 20 years of experience. We understand the physical and clinical demands of field-based therapy because we’ve been in your shoes. Urgent Need = Immediate Cases: Pick up patients and start working quickly.
Reliable Bi-Weekly Pay: Direct deposit every two weeks.
Provide high-quality, one-on-one physical therapy to geriatric patients in their homes.
Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge).
Create and implement effective plans of care (POC) to improve patient mobility and safety.
Coordinate seamlessly with our nursing and clinical leadership team.
Active Florida Physical Therapist (PT) License.
Home Health experience preferred (Kinnser/WellSky and OASIS experience is a plus).
