LPN Continuing Care

As a continuing care LPN, providing excellent patient care is essential. Under the direction of an RN, mid-level provider, or physician, you will assess, plan, implement, and evaluate patient care. You will play a vital role in the healthcare team, positively impacting the lives of patients and their families. You will collect and monitor assessment data and report on the physiological, psychological, sociocultural, and spiritual needs of patients and families. Direct patient care will be provided under the guidance of the RN or physician, consistent with policies and procedures. You will inform patients and families about treatments, medications, and equipment being utilized. Care provided will be documented accurately according to guidelines. Collaboration with other healthcare team members will be necessary to ensure continuity of care. Cleaning of the patient's environment, work area, and equipment will be part of your duties. You will document patient chargeable items according to standards. Participation in quality monitoring activities under the direction of the RN or physician is expected. You will advocate for the privacy, confidentiality, and security of patient, staff, and organizational data. Active participation in safety initiatives and risk mitigating measures is required.
