Social Work

Shift Time: M - F

Master\'s degree in Social Work (MSW) from an accredited program.

Experience: Previous experience as a Social Worker, especially in a clinical or healthcare setting, is preferred.

Capable of conducting home visits, traveling to various locations, and assisting clients with mobility when necessary.

Treatment Planning: Develop individualized care plans and treatment goals based on assessment findings, and implement evidence-based interventions and strategies.

Counseling: Provide counseling and therapeutic services to individuals and groups, addressing issues such as mental health, substance abuse, grief, trauma, and family dynamics.

Advocate for clients' rights and access to resources, services, and support systems, while collaborating with community agencies and organizations.

Case Management: Coordinate and monitor clients' care by connecting them to relevant community resources, healthcare providers, and social services.

Educate clients and their families about available resources, coping strategies, and skills to improve their overall well-being.

Maintain accurate and confidential client records, including assessments, treatment plans, progress notes, and discharge summaries, in compliance with legal and ethical standards.

Collaboration: Collaborate with multidisciplinary teams, including psychologists, psychiatrists, nurses, and educators, to ensure comprehensive care.

Community Outreach: Participate in community outreach and educational programs to raise awareness and promote mental health and social well-being.
