RN Home Health Shift Care

Registered Nurse - Home Health
Location: Wayne, Oakland, and Macomb counties, MI
Assignment Duration: This is a 17-week contract position with strong potential for conversion based on performance.
Schedule: Shift from 8 AM to 4:30 PM, with one Saturday every month.
Job Description: Under general supervision of the nursing supervisor, the registered nurse utilizes knowledge and application of nursing principles to provide direct patient care in the home. The role includes assessing the need for other allied healthcare services such as physical therapy, occupational therapy, nutrition, speech pathology, and social work to ensure an integrated home health care delivery system.
Responsibilities:

Provide direct patient care in the home environment.
Assess patient needs and coordinate with allied health services.
Manage a patient load of approximately 6 visits per day.
Drive an average of 40-60 miles per day to visit patients.
Required Experience / Certifications / Licensure:

Current RN licensure in Michigan.
BLS certification through AHA.
A minimum of one year of Home Health RN experience.
Current valid driver's license and reliable transportation.
Successful completion of an accredited registered nursing program.
