Diagnosing and managing acute, episodic, or chronic conditions in partnership with physicians or independently (contingent on your credentialing)
Engaging in patient follow-ups and ongoing chronic care management
Collaborating with interdisciplinary teams to facilitate seamless, holistic care
Accurately documenting interactions and care in electronic medical records (EMR)
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
Completion of an accredited Nurse Practitioner program
Active Nurse Practitioner licensure in the state of Arizona
National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or an equivalent specialty
Outstanding communication skills with patients, families, and fellow healthcare staff
Comfort using EMR systems and telehealth technologies
