Licensed Practical Nurse (LPN) (Full and Part Time, All Shift

We are currently seeking a Part-Time LPN/Licensed Practical Nurse to join our enthusiastic team at The Hearth at Drexel!

Shift/Schedule Available

Part Time

12-hour shifts

Direct the day‑to‑day functions of household staff or other nursing personnel in accordance with current rules, regulations, and guidelines that govern the Community.

Ensure that all assigned nursing personnel including household staff comply with the written policies and procedures established by the Facility.

Evaluate/ assess all residents with a change in condition daily, including suspected abuse.

Ensure physician/family notification and documentation of changes in condition.

Ensure proper procedures and policies for assisting with medications including accuracy, documentation, orders, etc., Responsible for reviewing all incidents on the shift of occurrence and follow‑up visits when deemed necessary. Shift documentation and visual assessment are required for nine shifts.

Ensure adequate supplies are available to staff at the beginning of shift.

Follow complaint procedures for all complaints using proper forms.

Three (3) years’ experience in geriatric nursing and supervisor experience preferred.

Must possess a current, unencumbered, active license to practice as a LPN in this state.
