This full-time position supports patients in Kansas City, MO and surrounding areas and is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year.

Responsibilities Lead and collaborate to provide management and delivery of patient care plans for home health patients.

Support patients in Kansas City, MO and surrounding areas as part of a team-based care approach.

Coordinate with schedulers and clinical educators to ensure timely and quality patient care.

Utilize tablets, mobile phones, and EMR software to support patient care activities.

Qualifications RN license in the state you work.

Graduate from an approved school of practical nursing.

One year of RN experience and the clinical competence to deliver quality patient care.
