Description

Conduct patient evaluations in either inpatient or outpatient settings to develop personalized treatment plans
Deliver physical therapy interventions focusing on mobility, strength, balance, and functional improvement
Engage with nurses, physicians, case managers, and other rehabilitation team members to enhance patient outcomes
Educate patients and their families regarding therapeutic objectives, discharge planning, and home exercise programs
Maintain records of evaluations, treatments, and patient progress per facility and regulatory standards
Participate in interdisciplinary team rounds and conferences to ensure cohesive care
Education
~ Doctorate in Physical Therapy (DPT) or an equivalent degree granted by an accredited institution

Certifications
State licensure as a Physical Therapist
CPR certification preferred
