Advanced Practice Provider (APP) - Transplant Outpatient
The University of Illinois Hospital & Clinics (UI Health) is seeking a full‑time Advanced Practice Provider (APP) to join their team. The APP will be an Advanced Practice Registered Nurse Certified Nurse Practitioner (APRN‑CNP) or Certified Physician Assistant (PA‑C) and will work as part of the multidisciplinary health team, providing high‑quality, cost‑effective advanced care to patients.

Duties & Responsibilities
Scope of Practice

Work within the bylaws of the University of Illinois Hospital & Health Sciences System and in adherence to hospital and divisional policy.

Collaborate with other providers in patient care.

Perform accurate histories and physical examinations, comprehensive or episodic health assessments, and develop patient care plans.

Perform procedures as authorized by clinical delineation of privileges.

Provide prevention strategies and manage acute and chronic health problems.

Order and interpret tests, treatments, and therapies within scope of practice.

Prescribe medications, diet, supplies, prosthetic devices, and other home health aides.

Perform other clinical duties as directed under the emergency operation plan.

Demonstrate fiscal responsibility to positively impact the organization’s financial performance while ensuring optimal patient care and safety.

Professional

Integrate evidence‑based knowledge and clinical expertise in decision‑making.

Participate in peer review and competency assessment activities.

Establish effective relationships with the entire care team.

Lead and assist in research and evidence‑based practice through participant selection, protocol coordination, data collection, and related activities.

Demonstrate accountability for professional conduct.

Pursue opportunities to develop new skills and enhance current ones.

Teaching

Provide comprehensive teaching strategies for patients and families.

Educate and mentor undergraduate and graduate students and new employees, including medical staff, residents, advanced practice providers and nursing.

Offer formal and informal education or mentorship to internal and external groups pertinent to the specialty.

Communication & Leadership

Participate in program development.

Act as a resource for clinical staff in the practice setting.

Consult with management on assessment, problem‑solving, decision‑making, and evaluating clinical/system issues and care models.

Identify and participate in quality‑improvement initiatives within the practice setting and initiate change as appropriate.

Lead multidisciplinary teams to provide integrated delivery of patient care services.

Participate in the development and revision of protocols or guidelines to reflect evidence‑based changes in care management.

Engage in activities that influence policy‑making bodies to improve patient care.

Perform other duties as assigned.

Minimum Qualifications

Graduation from a nationally accredited APRN or PA training program.

Current National Certification as Acute Care Nurse Practitioner (ACNP‑BC) or Adult‑Gerontology Acute Care Nurse Practitioner (AGACNP‑BC) by the American Nurse Credentialing Center (ANCC) OR current PA‑C certification by the Commission on Certification of Physicians Assistants (NCCPA).

Current license as an APRN‑CNP or PA‑C in the State of Illinois.

American Heart Association Basic Life Support (BLS) certification.

Controlled Substance licensure and Federal DEA registration.

Effective oral and written communication skills facilitating therapeutic relationships among physicians, APRN, PA providers and multidisciplinary care team.

Demonstrated experience in leadership, multidisciplinary collaboration, interpersonal communication, education, and clinical experience.

Ability to meet all requirements for medical staff privileges.

Preferred Qualifications

Nursing experience in a transplant department and/or transplant quality team.

Knowledge of transplant quality regulations.

Salary & Benefits
Salary Range: $128,818 to $157,730. Final salary will be determined by market data, internal equity, candidate experience, and budget constraints.
