LVN Shift Care for Home Health

An ambulatory care health center in Brooklyn, New York is seeking a Licensed Practice Nurse to provide patient-centered nursing support in a busy outpatient setting. The role involves delivering care for adult and pediatric patients through clinical procedures, patient screening, documentation, and coordination with the care team.

About the Opportunity:

Shift: Day

Schedule: Monday to Friday

Hours: 9:00am - 5:00pm

Setting: Ambulatory Care Center

Responsibilities:

Provide nursing care to assigned adult and pediatric patients under supervision in accordance with established standards and practices

Complete patient intake, including screening, scheduling support, and visit preparation for clinical appointments

Perform vital signs, venipuncture, injections, EKGs, and other routine clinical procedures as directed

Assist physicians and nursing staff with patient care, treatment procedures, documentation, and follow-up activities

Prepare exam rooms, monitor supplies, maintain a safe and clean environment, and help coordinate patient flow throughout the clinic

Qualifications:

• 1+ year of related Nursing experience
• Graduate of a Practical Nursing program
• NYS Licensed Practical Nurse (LPN)
• Experience providing care for both adult and pediatric patients
• Ability to communicate effectively in English, both orally and in writing

Desired Skills:

Bilingual

Experience in a high-volume, fast-paced outpatient setting.

Strong interpersonal and communication skills.
