Job Title: Licensed Practical Nurse

Service Location: Forest City, Pa; Shift: Weekday and weekend shifts, full-time and part-time schedules available!

Rate: $28-$32 per hour

Immediate Need:

Forest City, Pa: Full-time days and weekends

Carbondale, Pa: Monday through Friday 6:30am to 3:30pm for home and school

Peckville, Pa: Full-time evenings and overnights, Weekend hours all shifts

Lake Ariel, Pa: Evenings and Weekends

Throop, PA: Monday through Friday 8:15am to 3:30pm school hours

Implement and document the plan of care to ensure quality and continuity of care for patient

Administration of prescribed medication, treatments and therapies

coordination of care

Reports any changes in the client condition to Clinical Manager and physician

Health promotion, teaching, and training of family members
