About the Position
Join a mission-driven healthcare team in Central Phoenix providing comprehensive primary care to patients of all ages in an integrated family medicine and behavioral health setting. This unique practice serves a diverse patient population, including individuals with vision loss, and is dedicated to delivering accessible, compassionate, whole-person care. We are seeking a Family Nurse Practitioner who is passionate about improving health outcomes and making a meaningful impact in the community.
Schedule

• Full-Time
• Monday-Friday
• 7:00 AM to 4:00 PM
• No evenings, weekends, or major holiday clinic hours

Responsibilities

Provide primary care services for patients of all ages
Diagnose and treat acute and chronic conditions
Perform physical exams, preventive screenings, and health assessments
Order and interpret diagnostic tests
Educate patients on wellness, prevention, and disease management
Deliver compassionate care tailored to the needs of individuals with vision loss and other underserved populations
Collaborate with behavioral health and interdisciplinary care teams
Maintain accurate EHR documentation- EMR-NextGen with AI Ambient Assistant

Qualifications Required

Current or pending Arizona Nurse Practitioner license
Family Nurse Practitioner certification
Three years of primary care experience
Bilingual English/Spanish
