Job Description\n CloudinfraIT Inc is seeking a travel nurse RN Hospice for a travel nursing job in Sunnyvale, California.Job Description & Requirements\n \n Specialty: Hospice\n Discipline: RN\n Duration: 13 weeks\n 40 hours per week\n Shift: 8 hours, days\n Employment Type: Travel\n \n # Travel Hospice RN – Admission Visits | California\n **13-Week Contract | Days | 40 Guaranteed Hours/Week**\n **Territory:** San Francisco County, San Mateo County & Santa Clara County\n ⏳ **Contract:** 13 Weeks\n **Shift Options:**\n * **Sunday–Thursday:** 10 AM–7 PM or 11 AM–8 PM\n * **Tuesday–Saturday:** 10 AM–7 PM or 11 AM–8 PM\n * Schedule may be flexible based on patient availability\n * **1 weekend per month**\n ⏱️ **Guaranteed Hours:** 40 Hours/Week\n **EMR:** Homecare Homebase (HCHB)\n **Holiday Coverage:** Required\n **Call:** None\n ### Requirements\n * Active California RN License\n * Minimum **3 year of Hospice experience**\n * Current **BLS/BCLS**\n * Strong assessment and care-planning skills\n * Ability to coordinate care with a multidisciplinary team\n * W-2 employment only\n ### Key Responsibilities\n * Admit patients to hospice services\n * Assess physical, psychosocial, environmental, safety, and developmental needs\n * Develop individualized plans of care\n * Document patient assessments, observations, and interventions\n * Coordinate care with the multidisciplinary team\n * Conduct follow-up visits as needed\n * Participate in quality assessment and performance improvement activities\n ### Assignment Details\n * 13-week travel contract\n * Day shift\n * Two schedule options available\n * One weekend per month\n * Holiday coverage required\n * No on-call requirement\n * Overtime after 40 hours/week\n * Mileage reimbursement provided\n * 1099 not accepted\n CloudinfraIT Inc Job ID #299843.
