Description
Conduct evaluations for patients within both inpatient and outpatient environments and formulate tailored treatment plans.
Deliver physical therapy interventions emphasizing mobility, strength, balance, and functional abilities.
Engage and collaborate with nurses, physicians, case managers, and additional rehabilitation professionals to maximize patient outcomes.
Educate patients and their families on therapeutic objectives, discharge processes, and home exercise routines.
Document assessments, treatments, and progress in accordance with institutional and regulatory standards.
Participate in patient care team rounds and interprofessional conferences to guarantee cohesive care.
Education
~ Doctorate in Physical Therapy (DPT) or an equivalent from an accredited institution.

Certifications
Required state licensure as a Physical Therapist
CPR certification is preferred.
