Licensed Practical Nurses LPN - 1st, 2nd, & 3rd Shift

Job Summary: The Licensed Vocational Nurse (LPN) in this long term care role provides direct patient care to geriatric residents, including medication administration and supervision of CNA staff. The LPN will work independently with strong critical thinking skills to deliver dignified and respectful care in a long term care environment without acute care or tracheostomy patients. A minimum of two years' experience in long term care, a valid NYS LPN license, and BLS certification are required.

Responsibilities:

Provide direct nursing care to residents in a long term care setting.

Administer medications according to prescribed orders.

Oversee and coordinate care delivered by Certified Nursing Assistants (CNAs).

Perform patient assessments and monitor resident conditions.

Maintain accurate and timely documentation of patient care.

Collaborate effectively with interdisciplinary team members.

Ensure care is delivered with dignity and respect to all residents.

Manage wound care for residents as needed.

Required Skills & Certifications:

Current Licensed Practical Nurse (LPN) license in New York State.

American Heart Association Basic Life Support (BLS) certification.

Minimum of two years' experience in a long term care nursing environment.

Strong critical thinking and independent decision-making skills.

Preferred Skills & Certifications:

Experience working with geriatric populations.

Prior supervisory experience over nursing assistants.

Familiarity with electronic health record systems such as EPIC.

Special Considerations:

Work includes rotating shifts covering day, evening, and night schedules.

Must be available to work weekends, holidays, on-call, and callback as needed.

Care environment excludes acute care and tracheostomy patients.

Scheduling:

Rotating shifts including day, evening, and night shifts (D/E/N).

Shift assignments determined by unit needs and staffing requirements.

Coverage includes weekends and holidays with on-call and callback responsibilities.
