This is a 13-week contract position.
Schedule
AM shifts, 5 days a week, with 8-hour shifts.
Job Description
We are seeking an experienced Registered Nurse (RN) to provide care in a home health setting. The ideal candidate will possess recent home health experience and be familiar with HomeCare HomeBase. You will be required to travel within a coverage area of approximately 60 miles, attending to patients' needs in their homes.
Responsibilities
Perform Start of Care assessments and routine visits.
Conduct Resumption of Care visits and recertifications.
Manage discharges and follow-up appointments.
Document patient care and coordinate with interdisciplinary teams.
Required Experience / Certifications / Licensure
Must have current RN licensure.
Minimum of 30 care points required, including specific values for various types of visits.
Recent home health experience is mandatory.
Experience with HomeCare HomeBase is required.
Overtime must be pre-approved by the Administrator.
