About the Role: The Advanced Heart Failure Nurse - RN plays a central role in the outpatient management of patients with complex cardiac conditions, including advanced heart failure, cardiomyopathies, and hypertrophic cardiomyopathy (HCM). The RN provides comprehensive clinical support, patient education, medication management, and ongoing care coordination to optimize patient outcomes and quality of life. This role is suited for an RN seeking a fast-paced, highly specialized environment with strong long-term therapeutic relationships with patients and families.

Job Overview:
The RN collaborates with cardiologists, APPs, and the multidisciplinary heart failure team to manage patients effectively in the outpatient setting.

Qualifications
Required:

BSN

Graduate of an accredited school of nursing required

Minimum of 3 years' experience, with at least 1-2 years in cardiology and/or heart failure patients

Ability and willingness to learn clinical information and criteria

Strong computer literacy and communication skills (Microsoft Excel, Word, Teams, PowerPoint)

Preferred:

Knowledge of standard of care treatment for heart failure

Strong cardiac background

Heart Transplant experience

Critical Care experience

Certifications and Licenses Required:

Active NJ RN license or compact RN license with NJ endorsement

BLS from the American Heart Association

ACLS from the American Heart Association

Scheduling

Day Shift

Full-Time, 37.5 hours

Monday - Friday; on-call and weekend/holiday hours as required

Essential Functions

Participates in health promotion programs for patients and families

Designs health maintenance and disease prevention interventions

Provides anticipatory guidance and counseling to patients and families

Evaluates patients and caregivers' support systems

Facilitates patient transitions between health care settings

Manages commonly occurring acute and chronic physical and mental health issues

Patient Management: Heart Failure

Identifies suitability for Heart Failure team management

Identifies diastolic and systolic heart failure and their management

Identifies different cardiomyopathies

Interprets diagnostic studies and hemodynamics

Ensures accurate medication reconciliation on admission and discharge

Provides HF education to patients and caregivers

Activates Heart Failure pathway and provides referrals

Collaborates with attending physicians, cardiologists, and other specialties as needed

Other Duties

Collaborates with the clinical care team in ambulatory settings managing advanced heart failure patients

Conducts office-based patient assessments and monitors disease progression

Monitors and follows patients with timely follow-up and escalation of care

Triage patient calls and messages, determining urgency and care coordination needs

Supports management of patients on Guideline Directed Medical Therapy (GDMT) including titration per care plan

Monitors labs and imaging, communicates findings to providers and patients as needed

Coordinates care and referrals, including prior authorizations and specialty programs

Provides individualized education on disease progression and lifestyle modifications

Maintains accurate, timely documentation and adherence to regulatory standards

Participates in quality and performance improvement initiatives
