CCBHC Mobile Crisis LPN
The CCBHC Mobile Crisis LPN will be part of a 24-hour crisis stabilization hub that includes enhanced 24-hour crisis services and a mobile crisis unit that provides crisis stabilization services to the identified populations. Provides and directs physical and mental health care of patients located primarily in the Crisis Diversion Center, but must be flexible to the needs of HCI including working in other areas of the organization such as Vigo County Outpatient or Inpatient Unit. Functions as a part of the interdisciplinary treatment team, in which patient care is prescribed.

Demonstrates the ability to conduct and monitor the nursing process, which consists of assessment, planning, implementation and evaluation of nursing care.
Conduct admission assessments and reassessments as indicated according to policy, procedures and standards of care.
Follow admission and discharge procedures.
Works closely with law enforcement and community agencies to provide immediate intervention aimed at reducing the risk of emergency room and psychiatric hospitalization.
Functions as a member of The Mobile Crisis team consisting of Crisis Diversion Specialists, on-call therapists and medical team.
After stabilization of the crisis situation, will assist in connecting the individual to appropriate level of care.
Obtain and transcribe correctly the licensed independent practitioner's (LIP) orders for admission to the inpatient unit.
Participate as a member of the interdisciplinary team.
Document in the patient's record according to policy and procedure.
Assist in the identification of patient and family educational needs.
Participate in the teaching of patient and family educational needs especially related to physical issues.
Provide daily information at shift change and treatment review/discharge meetings.
Participate in CQI activities as assigned.
Complete safety plans and internal referrals for continued services after crisis has been stabilized.
Implement crisis intervention techniques when dealing with emergency situations.
Provides crisis assessments under the supervision of the on-call psychiatrist. Gather clinical information and staff with on-call psychiatrist.
Serve as a resource to Hamilton Center staff and community regarding available services, referral sources, emergency detention procedures, etc.
Participate in cross-training as assigned.
Ability to demonstrate competent use of Electronic Medical Record
Assist with answering inquiring phone calls, respond to requests for information/referral sources and route calls as appropriate.
Review referrals from outside agencies for HCI's inpatient unit and staff accordingly with on-call provider's orders.
Maintain a high level of ethical conduct regarding confidentiality, dual-relationships and professional stature.
Keep supervisor informed of potential crisis situations.
Maintains basic computer skills and knowledge in order to access client information and/or update information.
Performs other duties as assigned
Minimum Qualifications/Requirements

Knowledge and skills regarding basic nursing care and general knowledge regarding preparation and administration of medications.
Good oral and written communication skills.
Basic computer skills
Certificates, Licenses, Registrations

Licensed by the State of Indiana as a Licensed Practical Nurse.
Maintain current crisis prevention/intervention and American Red Cross CPR/AED certification.
Maintain current Crisis Prevention Intervention (CPI) certification
Completes all required continuing education requirements in a timely manner.

Exposure to disturbed and potentially aggressive patients is possible. Stress ranges from low to intense depending on situation.

Work Environment Work is normally performed in a comfortable physical environment within a scheduled daily regimen with steady time pressure, multifaceted tasks, and ongoing interruptions.
Conditions of Employment

Completion of tuberculin screening no later than three days prior to first day of employment and annually thereafter
Completion of MMR, Varicella, influenza and coronavirus vaccine
Completion of a pre-employment drug screening and completion of post-employment drug or alcohol tests upon reasonable suspicion of use.
Completion of Center-wide orientation and ALL required paperwork prior to reporting for work
Demonstrated computer literacy through successful completion of pre-employment testing may be required.
Completion of HCI Commitment to Quality training, Trauma Informed Care, and Relias Learning.
Completion of OBHP modules within the first 90-days.
Attendance at all mandatory staff development and training
Successful completion of a six month on the job orientation period
Participation in payroll electronic deposit
Adherence to Compliance Program Plan
Satisfactory reference and background investigation checks.
Must maintain licensure with the IPLA.
