Duties: .

Conducting mental health screening assessment interviews utilizing symptom measures, documenting the assessments in Epic and using database, and presenting at weekly team meetings.
Providing psychosocial assessments.
Providing benefits counseling.
Development of appropriate disposition planning for each patient. Identifying and providing patients with appropriate mental health/social service referrals as needed.
Participating as part of a multidisciplinary team including both mental health and medical providers toward coordination of care.
Working closely with related social and community agencies.
Providing professional and appropriate interventions in crisis or emergency situations.
Completing appropriate documentation in the electronic medical record.
Workload - 25-30 (estimated) assessments weekly - to include new (initial) and current (monitoring) patients, MH/social service/External Provider referrals (up to 10 weekly) - estimation
Skills:
One (1) year of LMSW experience.
Experience working in a mental health clinic or hospital setting.
Strong preference for experience working with ptsd/trauma-informed
Dedicated to working with a population coping with persistent mental and medical health conditions;
Strong clinical skills - In-depth psychosocial assessment and MH screening experience to include risk assessment
Organized
Experience with disposition/case presentation
Highly conscientious
Education:
~ Master's Degree in Social Work.

Required Certifications & Licensure:
Valid Licensed Master of Social Work (LMSW) license issued by NYSED

~ This position is under the World Trade Center clinic. They are located off-site at 145 East 32nd Street, New York, NY 10016. It's a short walk from Bellevue.
