Full-Time Physical Therapist (PT) - Stockton, CA

Riverwood Healthcare Center is seeking a Physical Therapist to join our rehab team. In this role, you will support high-quality physical therapy services across Skilled Nursing setting, helping residents regain function, independence, and confidence in daily life.

Evaluates, plans, organizes, develops, and directs the Physical Therapy (PT) services for assigned patients/residents in accordance with current applicable laws and regulations, and as may be directed by the Director of Rehabilitation and/or Administrator, to assure that the highest degree of quality care can be maintained.

Current PT license

Experience as a therapist in a nursing, hospital, or other related medical facility.

Training in rehabilitative and restorative therapy practices

Plans, develops, organizes, implements, evaluates, and directs the physical therapy for assigned patients/residents in accordance with current laws and regulations that govern nursing care facilities

Meets with administration, medical and nursing staff, as well as other related departments in planning therapy services

Maintains treatment records, resident files, and progress notes as required

identifies and initiates appropriate physical therapy interventions

Performs assessments and re-assessments as needed on each assigned patient/resident

Interviews resident to determine interests and types of treatments needed

Participates in development and implementation of resident assessments (MDS) and care plans, including quarterly reviews

Assists in transporting resident to and from physical therapy room/area

Documents patient care in accordance with regulatory, licensing, payer and accrediting requirements

Educates patient\'s family/nursing staff/caregivers in follow-through programs

Maintains equipment and work area in a safe and clean condition
