Referral Coordinator - Telecommute
Mountain Empire Older Citizens
Mountain Empire Older Citizens operates a remote role focused on coordinating referrals and DME for participants in the PACE network. You will manage referrals, medical records, and communications with external providers using the electronic health record to ensure timely appointments and proper documentation.
The position includes reception/medical records duties and collaboration with IDT, Quality Manager, and Program Director to resolve issues and maintain service quality.
