Our coordinated, multidisciplinary approach to your healthcare enables us to send a team of professionals delivering the optimal experience and training for each patient's condition and presentation. Position Overview Provides Physical Therapy services to patients as directed by plan of care in line with the philosophy, objectives, and policies of the agency. Supervises Physical Therapy Assistants and maintains responsibility for the implementation of follow through of PT procedures.

Responsibilities Assists the physician in evaluating patients’ level of function, establishing treatment plan and goals.

Performs initial evaluations within 48 hours of referral and re-evaluations every 30 days, as appropriate coordinating plan of care with the physician as needed.

Revises plan of treatment and contacts the physician for change in plan of care.

Advises and consults with patient’s family and other health care staff.

Coordinates care with agency DON and other staff as appropriate.

Supervises the Physical Therapy Assistant.

Instructs patient and/or family in the use of equipment safe patient management and home therapy programs as needed.

Communicates effectively with patients, care givers, and families as well as agency staff.

Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.

Checks in on Physical Therapy Assistants every 4-6 patient visits.

Qualification Must be a licensed Physical Therapist by the state of Illinois.

Must have at least one-year Home Health Care clinical experience.

Car with required proof of insurance coverage.

Able to read and interpret instructions related to patient care.

Must be able to travel to prospective patient’s place of residence.
