Physician - Hematology and Oncology - Hematologist/Oncologist

Health System is looking for an Outpatient long term Hematology/Oncology provider who has interest in going perm and helping build the program to start ASAP!

Practice Setting: Outpatient Hematology/Oncology

Coverage will include two locations

Reason for Coverage: New service line

Dates of Coverage: ASAP - ongoing

Patient Population for Required Coverage: Adult, Geriatric

What Percentage of the Practice is Hematology %, Oncology %, Internal Medicine %?: Hematology 50%; Oncology 50% - 0% IM

Average Number of Patients Per Day - OUTPATIENT (Clinic/Office): 30 average

Average Number of Patients Per Day - HOSPITAL (Call/Rounding): 0

Clinical Area: Oncology - Outpatient Setting

Documentation System/EMR: EPIC

Estimated Credentialing Timeframe: With an IN license 60 days; without IN license 90 days

Required Procedures:

Mgmt. of VAD/CV Catheters

Bone Marrow Biopsy

Required Duties:

Participation on a Tumor Board

Mgmt. of Bone Marrow Transplants

Orders Tests

Blood Bank

Orders Images

Prescribes

Administration of Chemotherapy:

Physician

Nurse oncologist

Submission Requirements

Board Certification (provider must have AMBS/AOA board certification in one, and competency in the other demonstrated) - [REQUIRED]

Active IN State License or IMLC with LOQ in hand - [REQUIRED]

BLS - [REQUIRED]

Provider must be willing to commit to long term assignment (Minimum of 6 months) - [REQUIRED]

A Self-Query NPDB report is required at the time of presentation (Should be pulled within 30 days) - [REQUIRED]
