This individual would be based out of their home while traveling to and from patient homes for clinical care.Key ResponsibilitiesEvaluate patients' functional status, muscle strength, mobility, gait, and ROM to develop and direct individualized treatment plansInstruct patients, families, and caregivers on therapeutic appliances, PT goals, and relevant care phasesMonitor and report patient progress and condition changes to the physicianPrepare and submit clinical progress summaries and participate in discharge planningIdentify needs for additional home health services and refer accordinglySupervise Physical Therapy Assistants and, when PT is the sole skilled service, evaluate home health aide care every two weeksProvide consultation, staff in-services, and participate in peer review as neededQualificationsBachelor's or Master's degree in Physical Therapy from an accredited programActive state PT licenseMinimum 2 years of PT experience (home health/community experience preferred)Strong verbal and written communication skillsCurrent CPR certificationValid driver's license, insured vehicle in good working order
