Responsibilities

Provide comprehensive outpatient Family Medicine care for patients across the lifespan.

Diagnose, treat, and manage acute and chronic medical conditions.

Provide preventive care, routine physical examinations, health screenings, and immunizations.

Manage common conditions including diabetes, hypertension, hyperlipidemia, respiratory illnesses, and other chronic diseases.

Provide age-appropriate health counseling, wellness education, and lifestyle management.

Perform in-office procedures as appropriate to training and scope of practice.

Coordinate specialty referrals, diagnostic testing, and follow-up care.

Collaborate with nurse practitioners, medical assistants, behavioral health providers, and other members of the care team.

Maintain accurate, timely, and thorough documentation in the EHR.

Participate in quality improvement initiatives and population health programs.

Engage in community outreach and health education programs.

Support continuity of care and develop long-term relationships with patients and families.

Perform other duties as assigned.
