Registered Nurse (RN-PRN) - Hospice Home Care

The Casual Home Hospice Registered Nurse (RN) in Manitowoc County plays a vital role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses in the comfort of their own homes. This position focuses on managing and coordinating comprehensive hospice care plans that address physical, emotional, and spiritual needs, ensuring quality of life and dignity for patients and their families on an as-needed basis. Current and valid Registered Nurse (RN) license in the state of Wisconsin.
Minimum of one year of clinical nursing experience, preferably in hospice, palliative care, or home health settings.
Strong knowledge of end-of-life care principles and symptom management.
Ability to work independently and manage time effectively in a home care environment.
Valid driver’s license and reliable transportation to travel within SSRCH territory. Conduct thorough patient assessments to evaluate physical symptoms, psychosocial status, and care needs in the home setting.
Develop, implement, and regularly update individualized hospice care plans in collaboration with the interdisciplinary team.
Administer medications and treatments as prescribed, monitor patient responses, and adjust care plans accordingly.
Provide education and emotional support to patients and their families regarding disease progression, symptom management, and end-of-life care options.
Document all patient interactions, care plans, and clinical observations accurately and timely in compliance with regulatory standards.
Coordinate with other healthcare providers and community resources to ensure seamless continuity of care.
Respond promptly to patient and family needs, including after-hours and emergency situations as required by the casual schedule.
The RN utilizes clinical assessment and critical thinking skills daily to evaluate patient conditions and adjust care plans accordingly, ensuring optimal symptom control and comfort. Effective communication skills are essential for educating patients and families, coordinating with interdisciplinary teams, and documenting care accurately.
