Regional Registered Nurse

The Regional Registered Nurse ("RRN") is a critical player in coordination of health care services for the Care Design NY (CDNY) Care Coordination Organization/Health Home (CCO/HH) members. This team-based approach to care and provide health services to individuals with intellectual/developmental disabilities (I/DD), through active partnerships with individuals, their caregivers/families, community resources, and their physicians. Facilitates a strong, stable person-centered approach to holistic service planning within and across settings to achieve coordinated, high-quality care. The RRN is a critical part of the team supporting the members and families we support with access to, and ongoing support related to all health care supports and services. The RRN is a source of expertise related to all health care support and services and works collaboratively with all team members to ensure a holistic approach to care. Including CMS Medicare-Medicaid Plan, NYSDOH CCO, OPWDD Reporting Requirements, and other measures to assess CDNY performance and to improve the quality outcomes for the people supported by CDNY and PHP.

Provide effective communication to improve health literacy. Work with care managers to assist with the development of Life Plans based on mutual goals with the individual, individual's circle of support, and provider's plan as appropriate.

Function as a resource to support care managers who need assistance with complex or problematic health care issues with members.

Participate in complex care case reviews, Clinical Rounds, and other teams as needed to problem solve and assist those members most in need of a comprehensive and creative approach.

Assist in monitoring the outcomes from urgent care and emergency room visits, hospitalizations, discharge planning, and post-hospitalization plans to reduce further re-occurrences.

Support in the training of staff on health care issues, medications, and medication management and proactive practices.

Identify and collaborate when inadequate health care is provided to the member to come to a better level of care.

Ensure adherence to regulatory requirements such as CMS, NYS DOH Health Home, NYS OPWDD requirements.

A Registered Nurse licensed to practice in New York state required

• 2+ years of experience in health services preferred
• 3+ years of work experience in a managed care plan or health home is preferred
• Proficiency in navigating through electronic record software programs
