About This Role The Registered Nurse in Home Care provides professional nursing care to patients in their homes, focusing on individual needs through assessment, planning, implementation, and evaluation. This role is ideal for nurses seeking a flexible schedule and the opportunity to make a meaningful impact on patients' lives.
Highlights Location: Brick, NJ
Schedule: Part-time, Every other weekend
What You'll Do Provide professional nursing care to patients in their home.
Assess, develop, implement, and evaluate home nursing care needs.
Ensure compliance with physician orders and documentation.
Demonstrate holistic care for patients and their significant others.
Complete nursing assessments and medication reconciliations.
Collaborate with physicians on the Plan of Care.
Implement teaching strategies to meet established goals.
Act as a liaison among physicians and the healthcare team.
Supervise para-professional home health aide staff.
Maintain awareness of new standards and incorporate them into practice.
Requirements Education:
Associate's Degree in Nursing or Diploma Certificate in Nursing required
BSN preferred
Experience:
At least 6 months RN experience or 1 year of LPN experience, preferably in home care or hospice
Licenses & Certifications:
Current NJ State Professional Registered Nurse License
AHA Basic Health Care Life Support HCP Certification required
