Position Overview
We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout Lawrence and Morgan Counties. The ideal candidate will have strong clinical skills, excellent time management abilities, and a commitment to delivering high-quality patient-centered care in the home health setting.

Key Responsibilities
Provide skilled nursing visits in patients' homes.
Perform comprehensive patient assessments and monitor changes in condition.
Develop, implement, and evaluate individualized care plans.
Provide wound care and IV therapy in accordance with physician orders and agency protocols.
Educate patients and caregivers on disease management, medication administration, and treatment plans.
Coordinate care with physicians, therapists, and interdisciplinary team members.
Complete timely and accurate clinical documentation.
Ensure compliance with state, federal, and agency regulations and standards.

Required Qualifications
Active Alabama Registered Nurse (RN) License in good standing
Experience with wound care and IV care
Strong clinical assessment and critical thinking skills
Valid driver's license and reliable transportation
Ability to work independently and effectively manage a field-based caseload
Homecare Homebase (HCHB) experience
