Description

Conduct assessments for patients in inpatient and outpatient settings and develop tailored treatment plans
Implement therapeutic physical interventions aimed at enhancing mobility, strength, balance, and functionality
Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal patient outcomes
Educate patients and their families regarding therapeutic goals, discharge planning, and exercises to practice at home
Record evaluations, treatments, and patient progress in accordance with facility and compliance guidelines
Participate in interdisciplinary rounds and fully engage in team conferences to facilitate coordinated care
Education
~ Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications
State licensure as a Physical Therapist
CPR certification is preferred
Additional specialized certifications (e.g., geriatrics, neurology) are advantageous but not mandatory
