Job Description
We are seeking a dedicated Physical Therapist with a minimum of 2 years of experience in home health. The ideal candidate will have demonstrated expertise in start of care and revisit assessments. This role involves delivering high-quality physical therapy services to patients in their homes.
Responsibilities
Conduct comprehensive assessments and evaluations of patients' physical abilities and needs.
Develop and implement individualized treatment plans to improve patients' mobility and overall health.
Perform start of care and revisit visits as required.
Document patient progress and modify treatment plans as necessary.
Collaborate with other healthcare professionals to ensure comprehensive patient care.
Required Experience / Certifications / Licensure
Minimum of 2 years of experience in home health.
Active Physical Therapist license in Pennsylvania.
