Pride Health is hiring a Home Health Physical Therapist to support our client's home health services in Easton, MD 21601. Location: Easton, MD 21601 Coverage Areas: Caroline, Talbot, Dorchester and Queens Annes county Job Type: Contract Contract Length: 3 months (possible extension or conversion) Pay Range: $52/HR- $56/HR *Pay offered is based on experience, expertise, credentialing, and education.
Assess home environment for safety risks and fall hazards.
Complete required documentation including OASIS and physician orders.
Provide therapeutic exercises, gait and balance training, and functional mobility training.
Train caregivers on proper mobility and transfer assistance.
Coordinate care with physicians and interdisciplinary team members.
Participate in discharge planning and recommend ongoing care or services.
Master's (MPT) or Doctorate (DPT) in Physical Therapy from an accredited program.
Home Health experience required.
Active Physical Therapist license (state-specific), MPT or DPT.
Ability to travel daily to patient homes within assigned territory (50 miles radius).
Comfortable working in home-based care environments.
