The Roads Home Health Care is looking for a dedicated Home Health Registered Nurse (RN) to provide skilled nursing care to patients across South Florida. Working alongside a Case Manager, you will help plan, coordinate, and document care for an assigned caseload, delivering hands‑on patient care that meets each individual's needs. This is a rewarding opportunity for a home health RN who wants to make a real difference in patients' lives while working as part of a supportive clinical team.

Provide skilled nursing care in patients’ homes, following the individualized plan of care

Assess each patient's condition, updating and adjusting the plan of care as needed

Obtain and follow physician orders, reporting changes in a patient's physical or emotional condition promptly

Participate in case conferences to discuss patient progress and coordinate multidisciplinary care

Teach, supervise, and counsel patients and family members on nursing care needs

Active Registered Nurse (RN) license in the State of Florida

Minimum one year of acute care nursing experience

Prior home health experience preferred

Comfortable delivering patient care across the lifespan, from pediatric to geriatric patients

Strong verbal and written communication skills for coordinating with physicians, patients, and families

Basic computer proficiency for entering and managing patient documentation

Working knowledge of federal, state, and agency regulations governing home health care

Bilingual candidates are encouraged to apply

Experience with home health EMR or documentation software is a plus
