Rehabilitation Services Physical Therapist

The purpose of this job/position is to function as a member of the Rehabilitation Services patient care team by providing physical therapy services such as therapeutic exercise, gait training, manual therapy, biofeedback, patient/family education, and various other physical modalities to assist patients in improving strength, endurance, mobility, and flexibility and to decrease pain. Therapists are expected to supervise therapy interns and to assist with other educational and research activities conducted by the department or the hospital and meet hospital standards regarding documentation and productivity. Initial and maintenance of credentialing is required for work at Murray Scholls Clinic.

Maintain general good physical and mental health and self-care in order not to jeopardize the health and safety of self and others in the workplace.

Includes but not limited to maintaining organization of supplies and equipment, notifying appropriate personnel when cleaning or repair is needed, follows infection control policies, maintains knowledge of life safety, fire and electrical safety and disaster plan.

Considers the needs of patients, employees, medical staff, volunteers, vendors and members of the community in an informative, courteous and compassionate manner.

Demonstrates excellent customer service and communication skills in all interactions with patient and family, internal and external customers, including telephone, electronic and face to face situations

Addresses complaints and problems promptly and courteously, involving management when indicated.

After chart review, performs individualized skilled physical therapy evaluation, including general screening for indications for other therapy disciplines rehabilitation needs.

Establishes treatment goals including functional outcomes, quantitative measure and target dates. Provides physical therapy procedures in a highly skilled and competent manor including clear explanations to patients the purpose for selected procedures. Includes patient, family, caregiver training and education in treatment plan. Documents evaluation, treatment plan and goals, treatment modalities and procedures in compliance with OHSU, OPTA, OAR, CMS, DNV and other 3rd party standards. Develops and/or improves patient care programs for specialty area.

Competently and safely demonstrated use of therapy equipment for treatment modalities and procedures.

Assesses patients for discharge from care when goals met, functional progress plateaus, pt noncompliance with treatment plan or change in medical status.

Enters appropriate billing for care provided.

Facilitates patient scheduling therapy appointments with follow up section in Cadence, MyChart communication.

A license with the Oregon Physical Therapist Licensing Board as a Physical Therapist; Completed an accredited physical therapy educational program and be eligible to obtain an Oregon Physical Therapy license or temporary permit.

At the time of appointment must have at least a temporary permit

Positions with in person patient care: Recent experience in an Ambulatory Care Setting or intensive clinical affiliation.

Board certification (ortho, sports, neuro, oncology, geriatrics, etc.); Beaverton-Greystone, Center for Health and Healing, inbeded in clinics depending on business need, and pending credentialing OHSU Health Murray Scholls Clinic.
