Physical Therapist - Full Time - Home Health Care

Clinica Recovery LLC is excited to announce an opportunity for a dedicated and compassionate Home Health Physical Therapist to join our dynamic homecare team in East Brunswick, New Jersey. In this full-time role, you will be pivotal in providing therapeutic services to patients in their own homes, helping them to achieve and maintain mobility, strength, and independence. The ideal candidate will be an experienced Physical Therapist who is licensed in New Jersey and possesses a patient-centered approach to care.

You will work Monday through Friday from 9 AM to 5 PM, where flexibility and adaptability are essential skills in adjusting to varied patient needs and schedules. Perform comprehensive in-home evaluations to assess patients' physical conditions, movement capabilities, and needs, developing personalized care plans based on findings.

Implement and conduct therapeutic interventions such as personalized exercise programs, gait training, and balance retraining to improve patient mobility and functional independence.

Provide thorough education to patients and their caregivers on treatment programs, effective home exercise regimens, and the importance of adherence to prescribed therapies.

Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure an integrated approach to patient care and to achieve optimal outcomes.

Travel within the assigned service area to meet patients in their homes, ensuring that each interaction is both comprehensive and compassionate.

Our client population primarily includes adults and seniors recovering from surgeries, injuries, chronic illnesses, or disabilities. Many patients benefit from specialized physical therapy interventions to regain their independence after hip or knee replacements, strokes, or falls. Our goal is to empower each individual through tailored rehabilitation plans that are enabling and effective within their home environments.

A valid and current New Jersey license to practice as a Physical Therapist is required.

A Bachelor's Degree in Physical Therapy or an equivalent from an accredited program is essential.

A minimum of 2 years of experience as a Physical Therapist, with experience in home health settings being highly preferred.

A valid driver's license and reliable transportation are necessary, as travel to patient homes is an essential part of the job.
